Our Director of Nursing begins the process by performing an initial client Clinical Assessment and develops a tailored Plan of Care for clients requiring assistance with basic activities of daily living (ADLs). In addition, she conducts routine client visits to perform plan of care updates and service provider supervision.We maintain consistent contact with our clients, their family and their service provider team. Our coordinated homecare program was developed to provide our clients with superior personalized services that promote continuity of care and enhance the quality of life.
